Thomas Bieber erforscht die krankmachende Rolle von dendritischen Zellen bei Hauterkrankungen und Allergien. Schwerpunkt sind chronische Hautkrankheiten wie Neurodermitis, die anhand eines großen Registers und Biobank-Daten Projektes untersucht werden. Ziele seiner Forschung sind eine verbesserte Diagnostik und neue stratifizierte Therapieansätze im Sinne einer „personalisierten Medizin“. Ein besonderes Interesse gilt auch der Entdeckung von Biomarker und der Entwicklung von neuartigen Arzneimitteln.

Die Neurodermitis ist eine komplexe Erkrankung bei der zunächst eine inadequate Aktivierung des angeborenen und anschließend des erworbenen Immunsystems mit Entstehung von krankmachenden Immunglobulin E (IgE) postuliert wird. Als Folge treten chronisch rezidivierende Entzündungsreaktionen an der Haut mit möglicherweise tiefgreifender Auswirkung auch auf anderen Organen wie die Lunge oder das Kardiovaskuläre System. Die Hautkrankheit gilt als nicht heilbar, sie ist aber behandelbar, wenn man die Komplexität der Pathophysiologie und des Phänotyps besser verstanden hat. Thomas Bieber will aufklären, wie solche komplexen chronischen Hauterkrankungen genau entstehen und wie sie erfolgreich behandelt werden können. Er erforscht dabei vor allem die Rolle der dendritischen Zellen und deren Interaktion mit dem Mikrobiom der Haut.

Dendritische Zellen sind Teil des Immunsystems. Sie sind die erste Abwehreinheit und sollen Eindringlinge in den Organismus bekämpfen. Bei Neurodermitis oder Allergien geben sie jedoch das Signal, überempfindlich zu reagieren. Im internationalen Forschungsprojekt CK-CARE hat Thomas Bieber zusammen mit Kollegen in Deutschland und der Schweiz eine Bio- und Datenbank auf mit Proben von mehreren tausend Patienten, die an Neurodermitis und begleitenden Allergien leiden aufgebaut. Mithilfe der Daten- und der Biobank werden derzeit die Patienten nach sog. Biomarker-Profile in Gruppen eingeteilt, damit Therapien passgenau entwickelt werden können. Darüber hinaus wollen die Wissenschaftler Biomarker entdecken, anhand derer sich der Verlauf und die Komorbiditäten (z.B. Asthma) der Neurodermitis vorhersagen lässt.
